Senior Care at Home
Our comprehensive group of health care providers is able to treat minor illness to chronic diseases with thorough assessment, including ordering of diagnostic labs and imaging, and referrals to specialists as appropriate. We travel to private residences, assisted living facilities, board and cares, and skilled nursing facilities. We also admit and manage patients in the hospital setting.

Whether you require a referral to a Cardiologist or a Podiatrist, we schedule and coordinate all visits to required specialists
Medication Refills/Reconciliation
Our team of healthcare providers can prescribe and refill most medications you may need, while ensuring they are appropriate for your plan of care. We also coordinate authorizations with your pharmacy and insurance to ensure all your medications are received.
Hospital Admission/Management
Our team of board-certified hospitalists admit and manage patients in the hospital setting. Our hospitalists also co-manage patients in the Behavioral Health Unit.
Telemedicine
Mobile Labs and Imaging
Excell Integrated Medical Group coordinates laboratory blood draws, EKG/ECHO services, and imaging studies such as X-Rays and Ultrasounds all in the home setting. We also coordinate DME supplies and refer to Home Health services as clinically indicated. All results are discussed at length with patients.

Pain Management
We manage and treat all types of pain. Our group also has a team of board-certified Pain Management physicians who can consult on our patients’ cases in skilled nursing and assisted living facilities.
Weekly and monthly visits
We are able to coordinate and schedule follow up appointments on a weekly or monthly basis. This ensures all needs and concerns are addressed in a timely manner.
Preventative Care
The providers of Excell Integrated Medical perform yearly physicals, order routine tests, provide counseling on diet and lifestyle modifications, screen for depression, and educate on smoking cessation. Our goal is to promote a healthy lifestyle.
